    Epithelial tumors of the intestines are a major cause of morbidity and mortality worldwide. The colon (including the rectum) is host to more primary neoplasms than any other organ in the body. Colorectal cancer ranks second only to bronchogenic carcinoma among the cancer killers. Adenocarcinomas constitute the vast majority of colorectal cancers and represent 70% of all malignacies arising in the gastrointestinal tract. The small intestine is an uncommon site for benign or malignant tumors despite its great length and vast pool of dividing mucosal cells. The classification of intestinal tumors is the same for the small intestine and colon. Although small intestinal tumors are addressed first, the bulk of the discussion is devoted to colorectal neoplasia.

    Oly 2% of all malignant tumors occur in infancy and childhood; nonetheless, cancer (including leukemia) is the leading cause of death from disease in the United States in chi9ldren over the age of 4 and up to 14 years of age. Neoplastic disease accounts for approximately 9% of all deaths in this cohort; only accidents cause significantly more deaths. Benign tumors are even more common than cancers. Most benign tumors are of little concern, but on occasion they cause serious disease by virtue of their location or rapid increase in size. It is sometimes difficult to segregate, on morphologic grounds, true tumors or tissues that are present in abnormal locations. Examples of heterotopias include a rest of pancreatic tissue found in the wall of the stomach or small intestine or a small mass of adrenal cells found in the kidney, lungs, ovaries, or elsewhere. The heterotopic rests are usually of little significance, but they can be confused clinically with neoplasms. Rarely, they are sites of origin of true neoplasms, producing the paradox of an adrenal carcinoma arising in the overy.

    Malignant mesotheliomas in the thorax arise from either the visceral or the parietal pleura. Although uncommon they have assumed great importance in the past few years because of their increased incidence among persons with heavy exposure to asbestos. In coastal areas with shipping industries in the UNITED States and Great Britain and in Canadian and South African mining areas, up to 90% of reported mesotheliomas are asbestos-related. There is a long period of 25 to 45 years for the development of asbestos related mesothelioma and there seems to be no increased risk of mesothelioma in asbestos workers who smoke. This is in contrast to the risk of asbestos related bronchogenic carcinoma already high and is markedly magnified by smoking. Thus for asbestos workers those who are also smokers), the risk of dying of lung carcinoma far exceeds that of developing mesothelioma.

When fully developed, crohn disease is characterized pathologically by (1) sharply delimited and typically transmural involvement of the bowelby an inflammatory process with mucosal damage, (2) the presence of noncaseating granulomas and (3) fissuring with formation of fistulas.
